Berks food safety inspections found evidence of rodent activity at one place and backflow of sewage hazards at several restaurants

Berks County is divided into three jurisdictions: Reading, Muhlenberg Township and the rest of the county. Reading and Muhlenberg Township have their own food safety inspectors and the rest of the county is inspected by Pennsylvania Department of Agriculture inspectors.

The results will be posted every other week. All food vendors are inspected at least once a year. Other inspections can be triggered by a change of owner, a follow up if a facility had been out of compliance or by a complaint filed with the PDA.

If a food provider is labeled out of compliance, that means it has one or more violations that require a return visit by an inspector. The establishment is given time to fix the problem(s) and remains open for business.

Below are the results of inspections conducted between Nov. 8 and Nov. 21, 2023, that were filed in the state database as of Nov. 22 at 12:30 p.m.

Food dispensing utensil in the bulk food containers observed stored in the food and not with handle above the top of the food and the container. Corrected. Observed a buildup of residue/mold like substance on the pre-wash hose nozzle for the sink by the ice cream serving area.

Stonekeep Meadery Tasting Room, 564 Blandon Road, Fleetwood, Nov. 16: no violations.

The Henry P. Robeson House Beef & Ale, 124 E Penn Ave., Robesonia, Nov. 16: eight violations. Mechanical warewashing equipment had a grease buildup observed inside and must be cleaned more frequently. Observed clean food equipment and/or utensils in clean dish rack area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Also cutting board on bain marie placed back wet with old water underneath. Tableware for customer use is not wrapped so that only handles are touched by consumers. Food facility warewashing sink has a direct connection to the floor drain, creating possible backflow of sewage hazard and needs an air gap. Rear door located in the kitchen area of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Working containers in bar/kitchen area, used for storing *chemicals, cleaners* taken from bulk supplies, were not marked with the common name of the chemical. Corrected.

Wingstop #2460, 532 E Lancaster Ave., Shillington, Nov. 16: two violations. 2nd unisex toilet room is not provided with a covered waste receptacle for sanitary napkins. The floor / wall juncture in kitchen door way area and walk in cooler wall at freezer door is not coved and closed to 1/32 inch.

Dairy Queen #10296, 4399 Penn Ave., Reading, Nov. 15: three violations. Observed wet wiping cloths in buckets at counter area, not being stored in sanitizer solution. Corrected. The food facility does not maintain Food Employee Certification records as required. The food facility does not have the original certificate for the certified food employee posted in public view. This is a repeat violation.

Jim Neidermyer Poultry, Leesport Farmers Market Bldg 2, Leesport, Nov. 15: no violations.

Lilli’s Ranch House, 2738 Penn Ave., Reading, Nov. 15: six violations. An open employee’s beverage container was observed in cook line area, a food preparation area. Corrected. Observed deeply scored cutting boards not resurfaced or discarded as required. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Bain marie drawer unit on prep side of cook line has spilled liquids and food debris in bottom under drawers. Observed clean food equipment stored uncovered or not inverted. Ice bucket not inverted at ice machine allowing for standing water in bottom. Hot Water, as observed at cook line hand sink, is not under pressure during inspection. Faucet must be repaired. Observed a build up of trash under the prep table outside of the office. Corrected. Plumbing system not maintained in good repair – observed the handwash sink leaking at the dishwashing area. Mops are not being hung to air dry. Corrected. Observed used paint materials and an open bag od tile grout material being stored on top of unused pizza boxes. Boxes disposed of. Corrected.

McDonald’s #02385, 3710 Penn Ave., Sinking Spring, Nov. 14: three violations. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in the refrigerator located near the back hand wash sink. Corrected. Observed self-serve soda machines, in the customer dining area, with an accumulation of food residue on non-food contact surfaces. Corrected. Plumbing system not maintained in good repair – observed faucet for the 3 bay sink leaking at the at top connection.

Salsa Burrito, 4900 Shillington Place, Reading, Nov. 14: four violations. Overwrap tape on equipment bottom shelves must be removed. Also some labels left on pans on rack. The food facility does not have the original certificate for the certified food employee posted in public view. Mop sink has a hose attachment that is hanging below the flood rim of the sink and not an appropriate air gap. Kitchen ceiling vent area of the food facility is dust and in need of cleaning.

Redner’s Warehouse Market #93, 5400 Perkiomen Ave., Reading, Nov. 09: three violations. Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in the Bakery Walk in. Observed the cutting boards in the Produce prep room to be deeply scored and not easily cleanable. Observed the interior of the ice machine and the ice cart, a food contact surface, was observed to have food residue and was not clean to sight and touch.

Smokey Bones Bar & Fire Grill #7552, 2723 N. Meridian Blvd., Wyomissing, Nov. 09: five violations. The 3-compartment sink at the bar is damaged and is leaking water onto the floor. Observed loose dust/dirt on ceiling vents and some ceiling tiles in the side storage/prep room and in the kitchen area. Plumbing system not maintained in good repair – observed water leaking at the drain for the handwash sink in the back room. One of the plastic light shields in the walk-in cooler is damaged and is no longer providing proper protection. Working containers (spray bottles) in the bar and dishwashing areas, used for storing chemicals, cleaners taken from bulk supplies, were not marked with the common name of the chemical. Corrected.

St Joseph Medical Center, 2500 Bernville Road, Reading, Nov. 09: three violations. Pre-wash hose handle/nozzle in manual warewashing area observed with build up of filth and food residue, must be cleaned more frequently to prevent build up. Corrected. Wash hose nozzle for cleaning dish rack and carts observed to have a build of filth and food residue. Corrected. Observed single use containers in the sandwich and coffee area in the front area of the cafeteria that were not stored inverted to prevent contamination of the food contact surface. Corrected. Paper towel dispenser empty at the handwash sink in the front hot food service area. Corrected.

Blue Marsh Restaurant & Pizzeria, 5027 Bernville Road, Bernville, Nov. 8: four violations. Frozen vacuum packed fish was left in packaging and thawed in refrigerator which is a prohibited method of thawing. Corrected. Observed clean food equipment and/or utensils in wait station area, stored wet in a manner that does not allow for draining and/or air drying (wet nesting). Drink cups stacked while wet. Sent back to rewash and air dry. Rear door located in the pizza dough prep area of the food facility has a large gap and does not protect against the entry of insects, rodents, and other animals. Soap was not available at the handwash sink in the kitchen area. Corrected.

Boscov’s #004 (Fudge Stand), 4500 Perkiomen Ave., Reading, Nov. 8: one violation. Working containers used for storing cleaners taken from bulk supplies, were not marked with the common name of the chemical. Corrected.

Boyer’s Food Market #3596, 200 W. First St. Suite 3, Birdsboro, Nov. 8: three violations. Thermometer for ensuring proper temperature of equipment is not available or readily accessible in the hot/cold display case near the cash registers. Food facility does not have available sanitizer test strips to determine appropriate sanitizer concentration at the 3-compartment sinks in the produce and deli departments. Corrected. Double-door entrance located in the receiving area of the food facility has a gap at the bottom due to a deteriorated seal and does not protect against the entry of pests.

Panera Bread #6072, 4229 N. Fifth Street Highway, Nov. 20: two violations. Observed broken thermometer in under counter refrigerator. Corrected (replaced) on site. Walk-in refrigerator shelves soiled.

Subway 222 Plaza, 3225 N. Fifth Street Highway, Nov. 17: five violations. Food employee observed in sandwich prep area, not wearing proper hair restraint. Observed empty condiment bottles not stored covered or inverted. Observed soiled condiment dispenser and bread support in the hand washing sink. Hand washing sink must be used for hand washing only. Water fixture at hand washing sink is not secure. Requires a base gasket. The floor / wall juncture in dining area has several feet of broken tiles and therefore is not coved and closed to 1/32 inch. Adjacent business damaged wall space between businesses.

Sammy’s Poultry, 2934 N. Fifth Street Highway, Nov. 11: three violations. Food employee observed not wearing proper hair restraint, such as a net or hat. Food contact surface of metal shelving in walk-in refrigerator is not smooth, easily cleanable and/or resistant to pitting, cracking or scratching. Ice machine interior panel was observed to be soiled.

Tacos Mexica, 2934 N. Fifth Street Highway, Nov. 11: four violations. Raw chicken was stored above raw beef, seafood in the undercounter refrigerator. Observed utensils with handles in different directions allowing eating portion of the utensil to be contaminated. Bain marie food (cheese) was held at 43.7 °F rather than 41°F or below as required. Lid closed and temperature dropped to 41 degrees F. Refrigerator gaskets on Undercounter thermometers/bain marie are soiled.

The following reports for the period of Oct. 25 to Nov. 7, 2023, were added to the state’s database after Nov. 9 and did not appear in the previous Berks food safety report.

Joe’s Kwik Marts (#1045) Exxon, 2855 Perkiomen Ave., Reading, Nov. 7: five violations. Observed two gallons of milk offered for sale beyond the sell by date of 11/6/23. Also observed 4 half gallons and 2 16oz bottles of milk offered for sale beyond the sell by date of 11/6/23. Corrected. Some soda nozzles at the soda unit, food contact surfaces, were observed to have residue and were not clean to sight and touch. Corrected. The following areas/surfaces were observed to have dirt/residue and are in need of cleaning: (1) counter under the self-serve drink units, (2) inside the cabinets under the self-serve drink units, & (3) floor/shelving in the side room (near the drink units). The food facility does not have the original certificate for the certified food employee posted in public view. Food facility inspection indicates evidence of rodent activity in some areas. Severely dented, swollen, distressed canned items observed in the canned fruit aisle are and intended for use or sale in the food facility. Corrected. Food facility does not have available sanitizer test strips or test kit to determine appropriate sanitizer concentration in the meat area. Corrected. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed heavy dust and dirt build up on the condenser fans in the deli walk in cooler and the produce walk cooler. Observed a build up of trash and food that has fallen under the shelves in the grocery freezer area. Corrected. Aztlan Mexican Grill, 1665 State Hill Road, Wyomissing, Nov. 6: three violations. Observed dirt/debris on the floor and shelves in the front and back areas. Mop sink in the back area is not draining properly. Observed standing dirty water in the mop sink. Food facility inspection indicates evidence of rodent activity in front and back areas. Facility needs to continue with their pest control program.

Blue Mountain Elementary, 45 Woodland Terrace, Hamburg, Nov. 6: no violations.

Breakfast Hut, 4170 Perkiomen Ave., Reading, Nov. 6: four violations. Food dispensing utensil in the bulk food bins observed stored in the food and not with handle above the top of the food and the container. Corrected. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed heavy dirt and food residue build up under the equipment on the cook line. Lights are not shielded or shatter proof in the dry storage area. Working containers in storage area by the dishwashing area, used for storing cleaners taken from bulk supplies, were not marked with the common name of the chemical. Corrected.

Grace Evangelical Lutheran Church, 30 Liberty St., Reading, Nov. 6: no violations.

Hocaa Bubble Tea, 1665 State Hill Road Unit Ks6, Wyomissing, Nov. 6: no violations.

Mikura Restaurant, 840 N. Park Road, Wyomissing, Nov. 6: four violations. Observed some open food products still being stored in the hibachi area of the facility. Owner disposed of all remaining open food items during this inspection. There is a hole in the floor/corner of the walk-in cooler (to the left of the door). Owner covered hole with a thick rubber material during this inspection. Kitchen items were moved to the hibachi area so the kitchen could be thoroughly cleaned. The hibachi area will need to be cleaned after the kitchen items are moved back to where they belong in the food preparation/storage areas. Sides of some equipment at the cooking line still have an accumulation of grease. Employee started cleaning these hard-to-reach areas during this inspection. Back door located in the dishwashing area of the food facility has a gap at the bottom and does not protect against the entry of insects, rodents, and other pests. Owner installed a piece of thick rubber material along the bottom of the door during this inspection.

La Salle Academy, 440 Holland St., Shillington, Nov. 6: two violations. Loose rubber door gaskets observed on the milk cooling unit. Observed deeply scored cutting boards not resurfaced or discarded as required.

Redner’s Warehouse Market #16, 500 Hawkridge Drive, Hamburg, Nov. 6: four violations. Food employees observed in produce prep area, not wearing proper hair restraints, such as nets, hats, or beard covers. Corrected. Gino’s Cafe, 400 W. Broad St. Unit D, Shillington, Nov. 3: two violations. Ceiling fan blades in rear kitchen area of the food facility are dusty and in need of cleaning. Romeo’s Pizza & Pasta, 30 W. Wyomissing Ave., Mohnton, Nov. 3: four violations. Food employees observed in kitchen area, not wearing proper hair restraints, such as nets, hats, or beard covers. Refrigerated ready to eat, time/temperature control for safety food prepared in the food facility and held for more than 24 hours, located in the walk in cooler, is not being date marked. Corrected. The food facility does not maintain Food Employee Certification records as required. A working container of *cleaner / sanitizer was stored on the same shelf with food in the grill area.

Bethany Children’s Home Main Kitchen, 1863 Bethany Road, Womelsdorf, Nov. 2:one violation. No sign or poster posted at the handwash sink in the kitchen area to remind food employees to wash their hands. Corrected.

Boyer’s Food Market #3580, 408 Park Road, Fleetwood, Nov. 2: one violation. Some food items were held at 83 and 93 °F, in the hot bar area, rather than 135°F or above as required. Foods were disposed of immediately even though they were not out of temperature for more than 4 hours. Facility will also have the entire unit serviced.

New bar warewashing sinks have a drainline that is below the flood rim of the floor drain and does not have an appropriate gap.

Observed three bags of dry food items stored directly on the floor in the back kitchen area, rather than 6 inches off of the floor as required. Corrected. The handwash sink in the back kitchen area was blocked by dishes and a large garbage can and was not accessible at all times for employee use. Corrected. Food facility 3-compartment sink has a direct connection to the floor drain, creating possible backflow of sewage. Facility has installed a plastic bottle on the end of the drain line for the sink, which has eliminated the air gap. Facility has one restroom toilet that is not working properly. Owner was in the process of attempting to clear a clog in the line during this inspection. Facility will call for plumbing service if they are unable to clear the clog. Paper towel dispenser empty at the handwash sink in the back kitchen area. Corrected.
